Ponvory (Ponesimod) is a prescription medication used to treat relapsing forms of multiple sclerosis (MS), a chronic autoimmune disease that affects the central nervous system. It works by selectively targeting and modulating the activity of certain immune cells, helping to reduce the frequency of MS relapses and slow the progression of disability in people with the disease. Ponvory is an oral medication, and it is typically used when other disease-modifying therapies have not been effective or are not tolerated.

Directions for Use

  • The usual starting dose is 20 mg once daily, with the dose gradually increased to 20 mg over a 14-day titration period. The dose is usually maintained at 20 mg per day after the titration phase. 
  • Ponvory should be taken orally with or without food. The tablet should be taken whole with a glass of water, and it is essential to follow the prescribed dosing schedule. 
  • If you miss a dose, take it as soon as you remember, unless it's nearly time for your next dose. Do not take more than one dose to make up for a missed dose. 

Safety Concerns

  • Ponvory may cause a temporary decrease in heart rate (bradycardia), particularly after the first dose. Patients are often monitored for at least 6 hours after the first dose to observe for any heart-related issues.
  • Ponvory may increase the risk of infections by weakening the immune system. Infections like respiratory infections and shingles may occur more frequently during treatment.
  • Liver function should be monitored regularly, as Ponvory can lead to elevated liver enzymes.
  • Ponesimod may cause eye-related issues, including macular edema (swelling in the central part of the retina), which can affect vision.
  • Ponesimod can harm an unborn baby, so it is important to avoid pregnancy while taking the medication. Women of childbearing potential should use effective contraception during treatment and for 1 week following discontinuation. 
  • Ponvory may lower white blood cell counts, increasing the risk of infections.

Avoid Ponvory (Ponesimod) If

  • Do not use Ponvory if you have a history of heart problems, including heart attack, stroke, certain arrhythmias, or severe bradycardia (slow heart rate). 
  • Ponvory is not recommended during pregnancy due to potential harm to the fetus.
  • It should be avoided in patients with severe liver impairment or active liver disease.
  • Ponvory should not be used in individuals with active infections due to its immune-suppressing effects.
  • People with a history of macular edema (swelling in the eye) should avoid using Ponvory.
  • Individuals with known hypersensitivity to ponesimod or any of its components should not take this medication.
